Six or twelve months after go-live, the programme has been declared a success and the vendor has moved on. This is the period in which the small losses accumulate. The template is the structure I use to look for them: interview prompts for managers, corridor questions, dashboard-reading protocols, and a writing scaffold for the report itself.
It is designed for a single practitioner to work through in about a week, alone or alongside a small internal team. The output is a short, readable document — not a dashboard, not a deck, not a traffic-light grid.
